IMMUNIZATIONS: Please be sure MDS has a current copy of your child's immunization record or a current copy of their Immunization Exempt form. This is a state regulation. Please turn in your child's immunization record or Immunization Exempt form as soon as possible.
STUDENT HEALTH FORMS: The Health Forms SENT HOME IN THE Back to School Packet help us provide nursing care to your child. Thank you for filling them out. We need the forms complete and signed by September 1, 2017.
MEDICATIONS: If possible, all medications should be given at home. However, if your child requires medication during the school day, a Medication Administration Form must be filled out and returned to the nursing staff to ensure safe medication administration. The form is located in the nurse's office and online under the On Campus tab on the left side of the Health and Well Being page under Health Forms. We cannot administer medications without a Medication Administration Form. This includes over the counter medication Please fill one out when you bring your child's medication to the nurse's office. We need you to fill out one form per medication.
SICK KIDS: Sick children belong at home. Help us keep your children healthy by keeping sick kids at home. Children with a temperature of 100° or more or children who are vomiting or have diarrhea need to stay at home for at least 24 hours.
CONTAGIOUS EXPOSURES: Please notify the nursing staff of any contagious diseases, such as chicken pox or Strep Throat. Information will be sent out via e-mail to all who may have been exposed.
Please feel free to contact our office any time with comments, concerns, or ideas you have for us to better promote health and wellness for your child while at school. Our email is nurse@manzanodayschool.org. Thank you.
